Supports adults aged 55 and older through social prescribing: identifies personal health and wellness goals, connects participants to non-clinical community services, and provides regular check-ins. Eligibility includes experiencing or being at risk of frailty, social isolation or loneliness, barriers to community resources, frequent emergency department or primary care visits, life-limiting illness, end-of-life, or palliative care. Eligible service areas include Nelson, Balfour, Bonnington, Taghum, Beasley, Blewett, and South Slocan. Accepts self-referrals and referrals from health professionals, family or friends, and community organizations.

Regional Health Connections transit route connecting Balfour with Nelson, Kaslo, and communities along the route.
BC Transit Health Connections Kaslo/Nelson provides transportation for non-emergency medical appointments. Medical trips have priority, but anyone may ride if space is available. Route 76 serves Balfour Ferry, Ainsworth Hot Springs, Woodbury Resort, Mirror Lake and Kaslo; the live timetable includes selected trips to Nelson and Kootenay Lake Hospital by request. Current published trips run Tuesday, Wednesday and Thursday, with trip-specific days and times; booking 24 hours ahead is required by phone at 1-855-993-3100. It is a community bus: standees are not allowed and bike rack use is limited to daylight. Confirm schedule and booking before travel.

The Province of British Columbia lists the Kootenay Lake ferry route between Balfour and Kootenay Bay, with a crossing time of about 35 minutes. The M.V. Osprey 2000 carries up to 70 vehicles and 144 passengers; the reduced-capacity M.V. Balfour carries up to 19 vehicles and 150 passengers during Osprey maintenance. The winter schedule lists ten daily departures from Kootenay Bay from 7:10 am to 10:20 pm. The summer schedule generally runs from the third week of June until after Labour Day and adds six M.V. Balfour departures, with first departure at 7:10 am and last at 10:20 pm. Financial assistance for eligible RDCK residents to reduce recreation admission, membership and program costs.
The Leisure Access Program (LAP) helps RDCK residents of all ages experiencing financial hardship access recreation. Approved participants receive a yearly subsidy credit of $100 per adult and $150 per youth. The credit can subsidize general admissions and memberships by 50% for adults and 75% for youth, and most registered programs by 50%; CHEER events are fully subsidized. A further allotment may be available in specified circumstances. Passes expire May 31 each year and applicants must reapply.

Nav-CARE is a free community-based program for adults living with chronic illness, social isolation, increased frailty or other quality-of-life concerns. Specially trained volunteer navigators provide practical, emotional and social support, help clients find local resources and activities, and offer consistent one-to-one companionship. No referral from a doctor or other professional is needed; clients and families can contact the program directly. Nav-CARE is not healthcare-system navigation.
