BCEHS Community Paramedicine provides scheduled, non-urgent care in patients’ homes and community settings. Depending on local program availability and assessment, services can include education, support and monitoring for chronic conditions; medication check-ins; vaccinations; home-health monitoring; wound care in coordination with nursing teams; wellness clinics and exercise groups; fall-risk checks; and connections to other health resources. The program focuses primarily on rural and remote communities, including older residents living with chronic or complex conditions. Speak with a physician, community health nurse, or nurse practitioner about referral. Contact BCEHS to confirm current Riondel program access and availability.
