Individualized non-medical support helping older adults connect with practical, social and health resources.
The Seniors Community Connector uses a social-prescribing approach to help older adults identify and reach personal health and wellness goals, with individualized planning, check-ins and links to community resources and programs. Eligibility: age 65 or older, residing on Salt Spring Island, and experiencing or at risk of frailty, social isolation/loneliness, or barriers to community resources.
