Team-based primary care for people in the Lower Columbia region who do not have a family doctor and have difficulty accessing care.
Provides primary care from family physicians and nurse practitioners. Eligibility: individuals and families in the Lower Columbia region without a family doctor who have difficulty accessing health care in their community. Patients must register and be attached before receiving services; this is not a walk-in clinic. Hours: Monday, Tuesday, Thursday and Friday 8:00 a.m.–5:00 p.m.; Wednesday 7:00 a.m.–7:00 p.m. Wheelchair accessible. Confirm current access and availability with the clinic.
