Free one-to-one volunteer support that helps adults with health or quality-of-life concerns connect with local resources and community activities.
Nav-CARE is a free community-based program for adults living with chronic illness, social isolation, increased frailty or other quality-of-life concerns. Specially trained volunteer navigators provide practical, emotional and social support, help clients find local resources and activities, and offer consistent one-to-one companionship. No referral from a doctor or other professional is needed; clients and families can contact the program directly. Nav-CARE is not healthcare-system navigation.
