One-to-one social prescribing and connection to community support for adults aged 55+ in Blewett and nearby communities.
Supports adults aged 55 and older through social prescribing: identifies personal health and wellness goals, connects participants to non-clinical community services, and provides regular check-ins. Eligibility includes experiencing or being at risk of frailty, social isolation or loneliness, barriers to community resources, frequent emergency department or primary care visits, life-limiting illness, end-of-life, or palliative care. Eligible service areas include Nelson, Balfour, Bonnington, Taghum, Beasley, Blewett, and South Slocan. Accepts self-referrals and referrals from health professionals, family or friends, and community organizations.
